VBS Volunteer and Participant Registration

We are so excited about Vacation Bible School this summer at Santa Fe Presbyterian! We are going to explore the story of Jesus' birth from some unique perspectives - a fun way to celebrate Christmas in July! There will be games, songs, crafts, stories, and all kind of fun! 

Date: July 15-17

Times: 9:00-11:30am

Ages: 4-years to finished 4th grade (older can attend as helpers!)

Cost: Free!

By submitting this registration, I confirm that I am either 18 years or older or the parent/legal guardian of the participant being registered. I voluntarily wish to participate in the Santa Fe Presbyterian Church VBS Program or to grant my child permission to do so. I understand that Santa Fe Presbyterian Church is a nonprofit charitable institution, which is voluntarily presenting this program for my child, other participants, and the community. I also understand that the program has activities that can involve physical contact with other participants, the ground or equipment, and that there is a resulting risk of physical injury to my child.

I understand and/or have explained these risks and benefits of participating in this program to my child and I/my child am/is in proper physical condition and have/has no existing injuries or conditions that could jeopardize my or his/her safety or health, or the safety or health of the other participants.

I therefore release and discharge all liability for any harm or injury suffered directly or indirectly as a result of my child's participation in the Santa Fe Presbyterian Church VBS Program, whether or not resulting from negligence, and I agree not to sue Santa Fe Presbyterian Church, its representatives, staff, or volunteers on any such claim. I also give permission for the staff, representative, or volunteers of Santa Fe Presbyterian Church to administer first aid or to seek medical care for my child during my child's participation in the program, including transportation of my child to a medical facility for additional treatment that appears necessary.
